SDOC Parents: Please read the following:
The Office of Special Education and Rehabilitative Services (OSERS) has instituted a ruling that requires districts to distribute written notification on an annual basis informing parents that under IDEA (Individuals with Disabilities Education Act) and FERPA (Family Educational Rights and Privacy Act) they are to obtain a one-time parental consent prior to disclosing personally identifiable information maintained in student’s education records. This notification is required in order to receive Medicaid reimbursement for health-related services provided to Exceptional Student Education (ESE) students and non-IEP students.
We must inform you that:
- This consent will not impact your child's Medicaid coverage;
- Consent can be withdrawn at any time; .
- Services listed in your child's IEP or other health plans will be provided at no cost to the parent or student;
- Services listed in your child's IEP or other health plans will be provided regardless of
your consent to bill Medicaid; - Upon request, you may receive copies of records disclosed pursuant to this authorization
