Non-Traditional Students

Dear Parent of Non-Traditional Student:
Any student who is registered for a Charter, Private, FLVS, Alternative and Traditional Public School Student is required to complete the FHSAA EL16 form. 

-- Please visit FHSAA and click on PARENTS (Top right side), then click EL16

-- Page 1 is to be completed by the student and his/her parent/legal guardian. The student and parent/legal guardian must sign in the presence of a notary public. Page 2 is to be completed by an administrator at the school the student attends. The documents must then be filed with the FHSAA member school at which he/she wishes to participate before the student can practice and compete. Page 3 is to be verified by the participating school. The participating school will review the form and attachments before registering the student with the FHSAA Office, utilizing the EL13 Form.
